other providers.The Home Dialysis Nurse Manager oversees Home
Hemodialysis (HHD) and Peritoneal Dialysis (PD) operations in the
dialysis unit, as well as supervises nursing personnel, organizes
and administers the Home Training Program, and implements ESRD
training and educational programs to ensure safe and efficient
dialysis treatments for all patients.Schedule: Fulltime Monday
through Friday, phone triage coverage as neededLocation: A
candidate does not have to live within the Clifton Park area and
can be located outside of the areaCompensation: Pay range from
$93,600-$115,000 annually, depending on nursing and dialysis

patient care, staff education and research projects in
collaboration with medical directors and DCI leadershipCreate and
employ strategies for growing home dialysisCollaborate with medical
director and clinical staff to assist patient in modality selection
and resolve care issuesAssist patient care team and home training
coordinator with responsibilities and substitute as neededOrganize
Home Therapies lobby days at in-center clinicsSupervise clinic care
team with medical director, including social work, renal nutrition,
technical and nursing teamsLead and participate in clinic governing
body related to home dialysisOversee patient care in accordance
with ESRD Network, regulatory agencies, DCIs Quality Assurance
Performance Improvement (QAPI) program and the individual clinics
requirementsSuccessful Candidates Bring:Excellent communication
skills and a desire to teachDemonstrated clinical excellenceStrong
leadership skillsDesire to collaborate with interdisciplinary care
teamAbility to problem solveEducation/Training:Accredited nursing
degree requiredCurrent NY Registered Nurse licenseMinimum one year
prior dialysis experience requiredMinimum six months experience in
home hemodialysis or peritoneal dialysis requiredOne year
