ClinicAlbany, NYWe are seeking a dynamic RN with excellent
attention to detail and clinical reasoning skills to join our
Community Cardiology outpatient clinic! Under the Supervision of
the Lead Nurse or Nursing Supervisor and Nurse Manager, the RN
Ambulatory provides nursing support to primary and subspecialty
providers. This is demonstrated by the use of the nursing process
to work collaboratively with the health-care team in planning,
directing, providing and evaluating patient care. The RN Ambulatory
is defined within the department to provide patient care, triage,
consultation and referral among team members. Essential Duties and
Responsibilities: Generally include the following. May vary at
clinical sites. Other related duties may be assigned:Completes
organized and quality documentation as required by the
department.Facilitates the utilization of resources to meet patient
outcomes (case management).Analyzes patient care needs and utilizes
a systematic approach to delivery of care.Provides age appropriate
direct patient care from the preparation of patients through the
end of the office visit and all tasks in between including
diagnostics, venipuncture, administration of medications, and
assistance of physicians as needed.Performs telephone triage
including coordination of care and documentation.Schedules tests
and procedures as indicated within the division and obtains prior
authorizations as needed.Works with physicians and other resources
to develop teaching tools for families and patients.Performs
community outreach as indicated.Work on studies with Principal
Investigator as indicated within the department.Provides patient
and family education taking into consideration cultural, religious,
language, and motivational, physical, emotional and cognitive
factors into account in the teaching planMaintain division medical
recordsUtilizes the Chain of Command as indicatedWorks with other
departments inside and outside of AMC to coordinate
multidisciplinary careEnsures that medical equipment functioning
properly and knows process to manage failure as need arisesCross
covers in other divisions within the department as
neededParticipates in the patient throughput process in the clinic
as indicated and needed (including vital signs, updating medical
records, performing diagnostic procedures, assisting the physician
and practitioners, facilitating teaching, end of appointment
coordination of care)Orientation of new staffWorks with division
physicians, administrative staff and Nurse Manager to assure
compliance with regulatory conditions and policies and
proceduresActively participate in the survey process in regards to
accreditationUtilizes scheduling system lookup to ensure
appropriate follow up has been scheduledLocates regulations and
policies and procedures on line and know how to applyUtilizes
educational resources on Intranet for self education and education
of other staff and familiesWrites letters of medical necessity as
indicatedDelegates daily tasks to appropriate clinical staff and
ensures completion of tasksCollaborates with administrative staff
and physicians on the Quality Improvement Team to address clinical
and non clinical issues within the divisionMaintains compliancy
with regulatory agenciesReviews and triages incoming test results
as indicated within the divisionDocuments properly, giving age
appropriate care, gathering data correctly (including vital signs),
and facilitating care in a customer friendly mannerMaintains
compliance with AMC standards for electronic system useMaintains
compliance with AMC Code of ConductMaintains a safe environment
within the clinic and working office spaceRealizes their role in
Environment of Care Surveys. Job Specific for Clinical Liaison also
includes:Works with Emergency Room team for all patient care follow
up Collaborates with Community agencies for optimal patient
careCollaborates with NYS for all executive physicalsCollaborates
with NYS for assuring VIP patients are followed through
AMCCollaborates with the Boxing commission for physicals
Qualifications:To perform this job successfully, an individual must

a state accredited Registered Nursing program is requiredCurrent
license to practice professional nursing in the State of New
YorkCurrent CPR certification requiredCertification in specialty
preferred Skills required for the Position:Strong customer service
orientationStrong communication skillsAbility to effectively
present information and respond to questions from physicians,
patients and their family members or other employees within the
work setting. Familiar with computer software. Effective Written
and Verbal Communication SkillsWorking Knowledge of MS Word and
ExcelExperience with electronic health record preferredAbility to
Maintain Confidentiality.A thorough knowledge of organization,
department and/or section policies and practices related to
Position preferredAbility to give medications via all routes and
ability to calculate medication doses.Proficiency in the operation
of standard office equipment (Adding Machine, Fax, Photocopier,
Credit Card Machine) and clinical equipment (Sphygmomanometer,
Electronic Vital Signs machine, pulse oximetry machine, infant
scale, adult scale and stadiometer), Type 45-60 WPM on a Typewriter
or Computer Work Environment (including Hazards): Clinical care
setting requiring ability to lift minimum of 10 pounds, extensive
walking and standing. May be subjected to infectious materials and
chemicals (i.e. Cidex). ***This is a best faith effort to clearly
