Job Description:
Conduct individualized nutritional assessments for outpatient
dialysis patients. ? Provide ongoing counseling and education
tailored to renal dietary needs. Monitor, interpret, and respond to
lab results to guide nutritional care. Develop and implement
nutrition plans of care in coordination with the interdisciplinary
dialysis team. Maintain accurate documentation of assessments,
progress notes, and patient education. Collaborate with
nephrologists and other healthcare providers to optimize patient
outcomes. Patient Population: Adults undergoing outpatient
dialysis. Patients located at both Cooperstown and Little Falls
dialysis centers. Work Schedule: Travel: Required between
Cooperstown and Little Falls dialysis sites Support Structure
