Job Description Job Description This position is eligible for up to a $15,000 Sign-on Bonus! Position Summary: The incumbent shall be responsible for providing a full range of physical therapy services including therapeutic interventions in a home environment consistent with the position’s qualifications. Education, License & Cert: Be a graduate of an accredited PTA degree program (or foreign equivalent). Current licensure in the state of primary practice is required. Experience: Experience working with an adult and geriatric population is preferred. Essential Functions: 1. Communicates and demonstrate a professional image/attitude for patients, families, clients, coworkers, and others. 2. Independent judgement and excellent planning and problem?solving skills are required. 3. Provide population appropriate therapeutic interventions resulting in positive outcomes as judged by quality monitoring and patient satisfaction. 4. Document patient response to treatment. 5. Document the course of patient care including progress made and continuing need for treatment. 6. Provide patient and family education on tasks and resources that will assist with continued functional independence. 7. Participating in continuing education/professional development activities. 8. Adhere to policies and practices of applicable professional organizations, entity facilities and Guthrie Home care specific policies while being familiar with and acts according to contents of applicable professional code, employee /handbook, unit Policy & Procedures. 9. Participate in performance improvement management activities. 10. Contribute to the delivery of cost?effective care, maintaining productivity while being cognizant of utilizing resources. Other Duties: 1. Travel for this position is always required. 2.
