Job Description:
Job Summary Responsible and accountable for the ongoing
teaching, data collection regarding the condition and care, safety
and monitoring of chronic hemodialysis patients as well as
providing support and education to the patients family. Functions
as an intricate member of the multi disciplinary team.
Responsibilities Maintains a therapeutic environment through
ongoing teaching, data collection and intervention in appropriate
response to the patients age, size and medical condition. Performs
and documents ongoing monitoring of the Dialysis patient.
Participates in patient care plans, development of educational
needs and evaluation of patient outcome. Reports changes in patient
care needs to the hemodialysis charge nurse or designee. Ensures
patient safety through recognition and appropriate response to
emergency situations, notifies RN, requests assistance as needed.
Sees that cultural practices will be addressed and supported as
long as such practices do not harm others or interfere with the
planned course of medical therapy. Performs safe, accurate and
timely administration of interdialytic medications. Contributes to
development of standards of care, policies and protocols, and
assists with QA/QI projects and data collection. Initiates,
monitors patient throughout treatment and terminates hemodialysis
treatment. Participates in patient and/or family conferences and
serves as patient advocate. Supports the philosophy, purpose and
objective of the Dialysis Department and the objectives of the
Patient Care Services Department. Performs any other duties as
assigned. Qualifications/Requirements EXPERIENCE 1 yr. Hospital
sub-acute experience and 1 yr. Hemodialysis experience preferred.
EDUCATION High School graduate and graduate from a Licensed
Practical Nurse Program LICENSES/CERTIFICATIONS Current NYS
Licensed Practical Nurse; BLS Certification
