ServiceINTRODUCTIONThe primary goal of the resource nurse is to
facilitate the provision and coordination of patient care on any
given shift and to assist staff in addressing patient care issues
and staff needs. The resource nurse plays a vital role in
supporting daily operations on the unit in collaboration with the
nursing leadership - DON and ADON. Some of these activities include
passing medications, completing treatments interdisciplinary
patient care rounds, coordination of residents' admissions,
transfers, and discharges, troubleshooting supply and equipment
issues, addressing grievances, monitoring infection control.Nurses
who function in the resource role demonstrate clinical competency
as well as an ability to prioritize, and think critically. The
resource nurse is an advocate for patients, families, and staff,
and integrates conflict resolution and negotiation into daily
practice. The resource nurse leads by example and utilizes unit
leadership to assist in problem solving, assessment and management
of patient, family, and visitor needs, administrative aspects of
the role (e.g. staffing requirements, scheduling) and exploring
personal professional development goals and learning needs.ROLE AND
RESPONSIBILITIES
- Medication Pass;
- Completion of Treatments;
- Conduct clinical audits on area facilities(s);
- Daily Documentation in Point Click Care (PCC);
- Picking up Orders in PCC;
- Notification of change - MD/Family/Designated
Representative;
- Completion of Accident/Incident Report, assisting with
investigation(s) as needed;
- Participation in staff, family and resident meetings as
needed;
- Participation in Wound Rounds as needed;
- Maintain focus on quality improvement;
- Report findings to the facility Director of Nursing and
Regional Director of Nursing;
- Meet with staff at the beginning of each shift to collect
pertinent information related to resident's acuity, discharges, and
other clinical issues or problems;
- Supervising CNAs and giving repot to them at the beginning of
shift (ensure showers are completed, skin checks are completed;
Call bells are within are in reach, adaptive in place etc.);
- Coordinate collaborative staff nurse decisions to address
staffing and nursing assignments;
- Coordinate communication from the clinical supervisor and other
intradepartmental staff as appropriate to facilitate unit
operations and resident care;
- Work with admitting and peers to coordinate admissions, patient
placement, and nursing assignments;
- Assisting with room changes as needed (ensuring that all
residents personal belongings are packed up appropriately to
prevent missing items and ensure smooth transition in move);
- Assist with patient care as needed which may include but is not
limited to: - ADLs - resident positioning/transfers - Contacting
interdisciplinary team to address resident problems;
- Monitor resident problems and clinical issues on an ongoing
basis and consult the appropriate resources in collaboration with
the DON/ADON/Facility Administrator/Facility NP/MD;
- Discuss discharge-planning needs with the admission team and or
case manager, social worker, and DON/ADON/Facility Administrator
during interdisciplinary morning;
- Facilitate discussion and resolution of staff conflicts and
when a breakdown in communication occurs;
- Consult with the nursing leaders related to clinical and
administrative issues;
- Serve as a back-up in emergency/critical situations to support
staffing and patient care activities;
- Collaborate with the DON/ADON/Facility Administrator to
coordinate staff attendance at unit-based in- services or other
internal educational programs as staffing permits;
- Check emergency equipment according to facility policy - Code
cart and defibrillator unit need to be checked daily;
- Oxygen/Nebulizer, BiPap, CPaP tubing must be changed and
labeled per facility policy and procedure.
- Med carts/treatment carts must be checked for expired/outdated
items and discarded/re-ordered when applicable.
- In the event of a disaster, the Resource Nurse will:
- Assure and updated to determine bed availability.
- Account for all staff on duty.
- Assess and identify residents for discharge home and/or skilled
nursing facility in collaboration with social services, medical
Team, and nursing staff.
- If an internal disaster, follow the facility emergency
preparedness manual guidelines.
- Maintain communication with regional nurse.
- Prepare to expedite acceptance of resident from the ED/or
inter-facility transfer.
- Other duties assigned by Clinical Leadership
Qualifications and Education RequirementsPREFERRED SKILLS
- 5+ years of work experience as a Licensed Practical Nurse in a
skilled nursing environment
- Active LPN license in good standing
- Excellent attention to detail
