Conduct the S.E.L.F. Trauma Informed Psychoeducational Group Curriculum with women living with or at high risk for HIV, STIs, and HCV. Develop and implement recruitment and retention strategies for a difficult to engage target population, including co-location at community and service provider sites. Provide HIV/STI/HCV testing services. Major Activities:* Adapt and tailor the S.E.L.F. trauma-informed psychoeducational group curriculum for women living with or at high risk for HIV, STIs and HCV, maintaining fidelity to the program model. Facilitate group sessions in a supportive environment.* Develop and implement strategies to recruit and retain the target population in the S. E. L. F. intervention and testing services. Establish and maintain relationships with service providers for co-location of services and bi-lateral referrals.* Conduct confidential HIV/STI/HCV testing and linkage in venues serving high risk women.* Participate in multidisciplinary case conferences to facilitate clients engagement and retention in care. Negotiate and advocate on behalf of clients for services that support self-sufficiency and self-management.* Collect, maintain, and report up to date client health indicator, contact, and history data. Utilize indicator data to address barriers and improve individual health outcomes. Implement strategies to communicate client health information among internal and external interdisciplinary teams.* Facilitate access and link clients to a continuum of Alliance for Positive Health and community services including PrEP and nPEP. Track referral follow-up.* Incorporate the Transtheoretical Model of Behavior Change and Trauma Informed Care strategies, as applicable, into all aspects of programming.* Maintain accurate records in accordance with New York state regulations and agency protocols. Maintain documentation, tracking and evaluation data in compliance with funder and agency requirements.* Incorporate best practices regarding confidentiality into all job duties and communications in accordance with HIPAA, Article 27-F, Alliance for Positive Health policies and procedures and other applicable regulations. Observe and abide by the HIV Confidentiality Law. Qualifications:* Bachelor's Degree in Social Work, Psychology, Public Health or related field; Master's preferred.* 2 years' experience with the target population of women from communities disproportionately affected by HIV, STIs, HCV and who often lack access to health and supportive services* Must have valid, unrestricted drivers license and a vehicle to travel to designated program sites, per program needs* Ability to work flexible hours as this position may require evening hours Knowledge, Skills and Abilities Required:* Knowledge of trauma informed care, behavior change theory, group dynamics, and evaluation methodologies* Good group facilitation skills, public speaking skills and presentation style* Strong interpersonal and communication (oral and written) skills* Ability to interact, work and communicate effectively with people of diverse backgrounds within a climate of mutual respect, inclusion, enrichment, and growth* Strong community liaison skills* Attention to detail; well organized; good time management skills* Ability to maintain professional boundaries within agency and grant guidelines, as well as with clients and external service providers
